Commercial Plan Review Instructions

Commercial Tap Application (Form 101)

Complete Commercial Tap Application (Form 101) and submit by mail (PO Box 348, Bowling Green, OH
43402) to the engineering department (attention: Stephanie Keefer). You can also email Stephanie the completed form.

Submit a completed Backflow Questionnaire to Emily Freeman or Simon Gundy, (PO Box 348, Bowling Green, OH 43402) or
email Emily or Simon the completed form.

Submit Plans

Submit by mail or hand delivery a set (full size) of site/survey plans showing detailed water, sewer infrastructure to the engineering department (attention: Stephanie Keefer) with applicable review fees of $200 each for water and/or sanitary sewer ($400 if both will be reviewed).

Plan Review

Plans will be reviewed by District staff. Comments/changes will be communicated to the owner/contact person.

Quote

The tap quote is prepared and subject to change based on the final plan review. The quote is emailed to the owner (or owner’s contact).

Pay Fees

If the owner elects to proceed, the owner submits fees based on the tap quote. All fees must be paid prior to water and/or sanitary sewer construction commencing.

Sanitary Sewer Tapper Information

To obtain a tapper license with the Northwestern Water and Sewer District (“District”), the District requires that you submit an application that includes a statement of your company’s experience, along with several references that the District may call

Upon submission and approval of the above-mentioned, you will also need to take and pass the District’s licensing examination. With an understanding of the District’s rules and regulations

If the above requirements are satisfactory, before the license can be issued to your company, the following must be submitted:

  • A bond for $25,000 for the current year with the Northwestern Water and Sewer District as the obligee.
  • A certificate of general liability insurance showing the Northwestern Water and Sewer District as the Certificate Holder, for the current year.
  • An application fee of $100 made payable to the Northwestern Water and Sewer District.

The sewer tapper license expires at the end of each calendar year. Therefore, the bond (or continuation certificate) and renewal fee of $25 will be required to renew at the beginning of the year to continue licensed status. Depending on when your company’s certificate of liability insurance expires, you will be required to provide the District an updated form to continue your licensed status as well.
